The recall was initiated after a consumer complaint reported that the nipple separated from the plastic shield, creating a choking hazard for young children.

The affected pacifiers were sold in three styles: the Mustache pacifier (model 00001), the Lip pacifier (model 0002), and the Smile pacifier (model 00003). These items measure 2.5 inches wide by 1.5 inches tall and feature a beige rubber nipple with two air holes. The back shield bears a label reading "MADE IN CHINA 1986 Paci-face, Inc. Patent pending." Pacifiers with a date code of "5/1/95" or later on the back of the blister card are not included in this recall.

The pacifiers were sold nationwide by Target Stores and other retailers between January 1994 and June 1994 for approximately $3.00. Consumers should immediately remove these pacifiers from the reach of young children and return them to the store of purchase for a refund.
