Mazda CX-5 Windshield Recall Due to Improperly Affixed Sensor Bracket

Pilkington is recalling replacement windshields for Mazda CX-5 vehicles because the Smart City Brake Support sensor bracket may loosen, potentially disabling safety features.

What this means for you

Real risk of harm even if no illness or injury has been reported yet. Stop using the product and follow the agency's guidance.

Our severity reasoning: The recall involves a defect that may disable active safety features (Smart City Brake Support), increasing the risk of a crash, which fits the rubric criterion for risk-of-harm products where injury has not yet been reported.

Plain-English summary

Pilkington North America, Inc. is recalling certain replacement windshields, model FW04026 GTY, manufactured between November 23, 2015, and December 16, 2015. These windshields are intended for use in Mazda CX-5 vehicles equipped with the Smart City Brake Support system. The affected units feature a bracket mounted on the inside of the windshield to hold a laser sensor or LIDAR device.

The defect involves the bracket being improperly affixed with only tape instead of the required combination of tape and urethane. This improper installation may cause the bracket to loosen or detach from the windshield. If this occurs, the safety features of the Smart City Brake Support system may be disabled, which increases the risk of a crash.

Pilkington will contact wholesalers and retailers that purchased the windshields to identify affected customers. The affected windshields will be replaced free of charge. This recall began on February 15, 2016. Owners may contact Pilkington at 1-844-278-7230 or the NHTSA Vehicle Safety Hotline at 1-888-327-4236.
