The affected heaters are 8 inches high and 6.5 inches wide, made of black ceramic, and come with a 6-foot cord. The front of the heater displays "MAXI-HEAT" next to power and safety indicator lights, with top-mounted controls for temperature and speed and a pull-up handle. A silver label on the bottom reads "ELECTRIC AIR HEATER, MODEL NO. H-415, MADE IN TAIWAN, UL LISTED." The serial numbers on the silver label range from 0026000 to 0031000. The heaters were packaged in cardboard boxes labeled "MAXI-HEAT SUPREME...CERAMIC HEATER...WITH AUTOMATIC THERMOSTAT CONTROL...MODEL H415."

Sam's Club stores nationwide sold these heaters for about $35 between September 1993 and February 1994. Consumers should stop using the heaters immediately and return them to King of Fans at 1951 N.W. 22nd St., Fort Lauderdale, FL 33311 for repair, replacement, or a refund. Consumers will be reimbursed for postage and should include a return address and specify their preferred remedy.
