Modera Pack N Play Mattresses Recalled for Entrapment and Suffocation Hazard

Modera is recalling about 15,000 Organic Cotton Pack N Play Mattresses due to improper fit in certain play yards, creating a suffocation hazard. Babies can become trapped between the undersized mattress and side walls.

What this means for you

Real risk of harm even if no illness or injury has been reported yet. Stop using the product and follow the agency's guidance.

Our severity reasoning: This recall involves a risk-of-harm product (entrapment and suffocation hazard) with no reported injuries or deaths. While the hazard is serious and based on violation of mandatory crib mattress standards, the absence of reported incidents places it in the High severity category.

Plain-English summary

Modera is recalling approximately 15,000 Organic Cotton Pack N Play Mattress Dual-Sided Portable Baby Crib Pads. The mattresses are sold in white, pink, blue, and gray with "Modera baby" printed on the labels. They measure 38 inches long, 26 inches wide, and 1.5 inches thick.

The mattresses may not fit properly in certain play yards and violate mandatory safety standards for crib mattresses. When undersized, gaps can form between the mattress and the play yard's side walls. Infants' faces can become trapped against the mattress and side, potentially preventing breathing.

The mattresses were sold on Amazon from April 2024 through June 2025 for between $30 and $44. No injuries have been reported.

Consumers should immediately stop using the recalled mattresses and contact Modera for a refund. The company requires consumers to cut the mattresses in half and send a photo to [email protected] to claim the refund. Consumers can reach Modera toll-free at 877-450-0320 (Monday–Thursday, 10 a.m.–4 p.m. ET; Friday, 10 a.m.–1 p.m. ET), by email at [email protected], or visit https://moderababy.com/recall.
