The specific model numbers involved are 439-A, 439-B, 439-G, 439-P, and 439-W, which can be identified by a tag located under the mattress pad.

The hazard exists because the wire supports on the sides of the bassinet can disconnect, causing the fabric sides to lower. This poses a risk that infants can fall out of the bassinet or become entrapped and suffocate. Dream on Me has received one incident report of the wire frame support bracket failing and the fabric portion collapsing while an infant was asleep, though no injuries have been reported.

The product was sold online at Amazon.com, Walmart.com, Wayfair.com, ToysRUs.com, and Kohls.com from May 2012 to October 2014 for about $60. Consumers should immediately stop using the product and contact Dream on Me to obtain a free repair.
