The hazard exists because the swivel could separate while a diver is underwater. This separation would result in a complete and sudden loss of the diver's air supply. Additionally, if the diver panics or the emergency procedure fails, there is a risk of air embolism or drowning.
The firm has received one report where the swivel separated during a dive, causing the diver to ascend using the buddy breathing technique. No injury was reported in this incident.
