The hazard involves the lamp falling out of the lamp socket and striking someone standing or walking beneath the fixture. If the lamp breaks upon impact, it poses a laceration risk. Guth Lighting has received reports of about 50 lamps falling from the socket, but no injuries have been reported.

The affected fixtures were sold nationwide through lighting and electrical supply distributors from January 1999 through October 2003 for approximately $350. The fixtures measure 22 inches in diameter and are labeled "GUTH LIGHTING, ENVIROGUARD LOW MOUNT" with specific catalog numbers starting with ELP-O 2. Affected owners will be contacted directly by Guth Lighting and their distributors to receive a retention plate to be installed over the lamps to hold them in place.
