The Colorforms puzzles are five-piece wooden items sold in brightly colored boxes labeled for ages 1 1/2 to 3 years. Some pieces are small parts that can be swallowed. The Crib Pals rattle measures 5 1/2 inches by 1 3/4 inches and contains a ball with liquid and pellets on one end and a squeaker on the other. The rattle can break during use, creating broken pieces that pose a choking risk. Approximately 12,000 puzzles were sold nationwide from September 1986 to December 1987, and approximately 24,000 rattles were sold nationwide from March 1986 to December 1987.

Although the CPSC and Toys-R-Us have not received reports of any injuries or deaths involving these products, consumers should take the toys away from children.
