The defect involves welded rod eyes on the cylinders that were not properly heat treated during the manufacturing process. An improperly manufactured rod eye end attachment may fracture or separate. If two affected cylinders are installed on the same aerial lift device, the aerial lift may retract without warning, which could cause severe injury and/or property damage.
