While the CPSC is not aware of any incidents involving this specific product, the agency is aware of one death involving a similar device manufactured by another company that tipped over and struck a three-year-old in the head.

These devices were sold through approximately 25 wholesalers to retail stores nationwide since May 1991. The retail price was approximately $49.95.
