The recall was prompted by a reported death and two severe injuries. A 29-year-old woman died in Andover, Massachusetts, after fracturing her neck when the slide partially deflated, causing her to hit the ground. Two other individuals suffered similar injuries, including a 24-year-old man who became a quadriplegic and a woman who fractured her neck. The hazard exists because the slide can deflate during use, allowing users to hit the ground, and it is unstable, capable of toppling over in still or windy conditions. The product also carries inadequate warnings and instructions.

Consumers should immediately stop using the product and return it to the nearest Walmart or Toys R Us for a full refund.
