An "O" logo is engraved on the tube cup under the rotary power switch, and the name "OTT-LITE" is engraved on the front of the base. The model number 13Z63BN9 or 13T63BN9 is printed on a black label on the underside of the base. These lamps were sold at Office Depot stores nationwide from May 2006 to October 2006 for about $80.
