The affected wall mounts are black, made of metal, and measure about 30 inches long and 18 inches tall. The center bar can extend out to about 13 inches. These mounts are designed to fit most 37-inch to 80-inch flat-panel televisions weighing up to 125 lbs. The words "Sanus Simplicity" appear on the top left corner of the box and the instruction manual, but not directly on the product. The model number is located on a label sticker on the metal wall plate, under the plastic cover that covers the top horizontal bar.
Milestone has received two reports of the television becoming detached from the arm assembly, including one report of a shoulder injury. The mounts were sold exclusively at Costco from July 2013 to September 2014 for about $125.
