The recalled slides, manufactured by Horizon Industries, feature single and double bedways with continuous welds attaching the bedway to the sidewalls. There are no identifying marks on the slides.

The recall is due to a hazard where the weld between the slide bedway and sidewall can crack and separate. This separation creates a space where a child's fingers can get caught, posing an amputation hazard. Playworld is aware of 13 incidents of broken welds, including two cases where children suffered finger amputations.

The slides were sold by independent distributors to parks, schools, and municipalities from November 2000 through October 2016 for between $1,500 and $4,000. Consumers should immediately stop using the recalled slides and contact Playworld for a free replacement slide and installation. A temporary barrier will be shipped to consumers before the replacement slide to prevent use.
