Gladys Ave. Chicago IL 60624, Model: Tight Top RB Sweet Dreams, Protoype ID: TTQRBD Sweet Dreams."

The recall was issued because the mattresses fail to meet the mandatory federal open flame standard for mattresses, which poses a fire hazard. No injuries have been reported in connection with this recall.

The affected mattresses were sold at independent mattress stores in Illinois, Indiana, and Wisconsin from September 2014 through December 2014 for between $50 and $500.
