The defect involves machine dimensions for the center cap and steel lug inserts that were not controlled to required specifications. As a result, the wheels may not be tightened to the proper torque during the mounting process. If the wheels are not tightened correctly, they could loosen, leading to wheel separation and a potential vehicle crash. A detached wheel also poses a hazard to other road users.
