The recall affects 3,547 units, including 3,063 beds and 484 motors, distributed nationwide in the United States, Puerto Rico, and internationally to the Bahamas, Canada, Chile, El Salvador, and Santo Domingo.
The defect is caused by contaminated material in the plastic housing of the OKIN America "DUOMAT 3" Head/Foot motor. This contamination may cause the motor housing to crack, resulting in a motor that fails to actuate the head section as intended. If the head section were to fall, an injury could occur, though this is described as unlikely but possible. No injuries have been reported.
Consumers should contact GF Health Products or OKIN America to arrange for the field correction. The affected serial number ranges are US0458/US0468 (697142145108 through 697142147927 and 697144140132 through 697144140172) and US6000 (69204141262 through 69204141737). Service motor kits with ship dates from May 4, 2015, through June 30, 2016, are also included.
