These units were installed in McDonald's Restaurant Play Places nationwide between February 2016 and October 2018. The climbers feature plastic platforms, ropes, cables, and chains for children to scale inside a multi-level enclosed tower assembly.

The recall is due to a fall hazard. The climbing level platforms were designed too far apart, which poses a risk to children who may fall while climbing from one level to the next. There have been 37 incidents resulting in 34 reports of injuries.

Soft Play has contacted all purchasers directly and installed additional platforms free of charge to reduce the distance between platforms.
