The recall covers Series 503 units manufactured from July 2009 through April 2011. These rectangular, black, two-piece vessels are installed inside toilet tanks and were sold individually to be installed in toilets from various manufacturers, including American Standard, Kohler, and Crane.

The recall is being issued because the systems can burst at or near the vessel weld seam, releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and causing property damage. Flushmate has received two reports of units bursting, resulting in property damage, including one report of a minor injury.

Consumers should immediately stop using the recalled systems, turn off the water supply, and flush the toilet to release internal pressure. They should then contact Flushmate to request a free repair kit. Affected units have a 16-character date code/serial number on the top label, with date codes ranging from 070109 to 043011.
