The name Q4 Designs LLC and style numbers U274-200 (sizes 4-6) or U274-300 (sizes 7-16) are found on the white tracking label tag sewn into the inside of the jacket near the waistband.

About 1,400 jackets were sold at Burlington Coat Factory, Kids Stop, and Cookie's Department Stores from August 2013 to October 2013 for about $15. The jackets were manufactured in China and imported by Samsung C&T America, Inc. Consumers should remove or cut the band to eliminate the hazard, or contact Q4 Designs to obtain a full refund.
