What this means for you

Illness, injury, or structural failure has been reported. Stop using the product immediately and contact the manufacturer for a refund or repair.

Our severity reasoning: The source text explicitly classifies the recall as 'High - Class I' and 'Class I'. According to the rubric, if the agency's own classification is FDA Class I or USDA Class I, the score must never go below 4. Additionally, the hazard involves a high-risk pathogen (E. coli O157:H7).

Plain-English summary

Skyline Provisions, Inc., a Harvey, Illinois establishment, is recalling 1,029 pounds of beef products contaminated with E. coli O157:H7, the U.S. Department of Agriculture's Food Safety and Inspection Service (FSIS) announced on April 27, 2015. The affected product consists of 17 boxes of Aurora Packers Intact Beef Round Flats produced between April 15 and April 25, 2015.

FSIS discovered the contamination during a routine sampling program. On April 15, 2015, Skyline sold the product under their D&S label (Establishment number: 19300), ground and tested one and a half cases of the product. On April 21, 2015, these products were found positive for E. coli O157:H7. The remaining intact products were sold to Jack & Pat's Old Fashioned Market in Chicago Ridge, Illinois, where the product was ground and sold in various amounts of ground chuck patties, ground chuck, ground round, sirloin patties, and porter house patties.

Neither FSIS nor the company received any reports of illnesses associated with consumption of this product. FSIS and the company are concerned that some product may have been sold and stored in consumers' freezers. E. coli O157:H7 is a potentially deadly bacterium that can cause dehydration, bloody diarrhea, and abdominal cramps. While most people recover within a week, some develop a type of kidney failure called hemolytic uremic syndrome (HUS). This condition is most common in children under 5 years old and older adults.

FSIS advises all consumers to safely prepare their raw meat products, including fresh and frozen, and only consume ground beef that has been cooked to a temperature of 160 °F. The only way to confirm that ground beef is cooked to a temperature high enough to kill harmful bacteria is to use a food thermometer that measures internal temperature. Consumers with questions regarding the recall can call Skyline Provisions, Inc., Chairman Frank Grove at (708) 331-1982.
